About the opportunity
The Métis Community Health Pathfinder (Social Worker) provides assessment, care planning, intervention, counseling and referral for services to Métis and Indigenous clients and their caregivers/families. The incumbent will use his or her skills, gifts and lived experience to support members of the local Métis Chartered community in their navigation of Primary Care Network (PCN) and social programs within the Port Alberni region.
The Métis Community Health Pathfinder is a valued member of the PCN team and will collaborate with PCN providers and partners to ensure that care provided to Métis people is culturally informed and that access to other PCN resources or other social programs is facilitated for Métis elders, adults, children, youth and families.
